Patient Information

Understanding Varicocele — Signs, Grades & Recovery

A varicocele is an enlargement of the pampiniform plexus — the network of veins that drain blood from the testicle. It typically affects the left side and is most common in men aged 15 - 35. Many varicoceles cause no symptoms, but some lead to discomfort, testicular shrinkage, or reduced fertility. Here's what every patient should know.

Common signs of a varicocele

"Bag of worms" feel

A characteristic soft, lumpy feel in the scrotum, especially while standing.

Dull scrotal ache

Heavy, dragging discomfort that worsens by the end of the day or with prolonged standing.

Visible scrotal enlargement

A noticeable lump or swelling on one side of the scrotum, often the left.

Reduced fertility

Found during fertility evaluation — varicoceles can lower sperm count and quality.

The three grades of varicocele

1
Step 1

Grade I — Small

Detected only with a Valsalva manoeuvre. Usually asymptomatic.

Observation if not causing symptoms or infertility

2
Step 2

Grade II — Moderate

Palpable on standing, not visible. May cause mild discomfort.

Microscopic or laparoscopic varicocelectomy if symptomatic

3
Step 3

Grade III — Large

Visible through the scrotal skin. Often heavy or aching.

Surgical repair recommended

Step 4

Subclinical

Detected only on Doppler ultrasound, not on examination.

Repair if linked to infertility on semen analysis

Advanced Varicocele Surgery Procedures We Offer

Microscopic Varicocelectomy

Microscopic Varicocelectomy

Microscopic varicocelectomy uses a high-powered operating microscope to identify and ligate every dilated vein while carefully preserving the testicular artery, lymphatics, and vas deferens through a small subinguinal incision. The precision approach causes minimal tissue disruption, leading to less post-operative pain, the lowest recurrence rates, and the lowest risk of post-operative hydrocele — making it the gold standard for varicocele repair, especially when fertility is a concern.

  • 30 - 60 minute procedure
  • Recovery in 1 - 2 weeks
Laparoscopic Varicocelectomy

Laparoscopic Varicocelectomy

Laparoscopic varicocelectomy uses 3 small keyhole entry points in the abdomen to access and ligate the dilated testicular veins higher up at their origin near the kidney. This minimally invasive approach is well-suited for bilateral varicoceles in a single session, since both sides can be addressed through the same keyhole access — and remains an established option with proven safety and efficacy.

  • 45 - 75 minute procedure
  • Recovery in 1 - 2 weeks

What to expect — consultation to full recovery

Day 0

Consultation, examination, scrotal Doppler ultrasound, and (in fertility cases) a semen analysis.

Day 1

Microscopic or laparoscopic varicocelectomy as a daycare procedure under general or spinal anaesthesia.

Day 1 - 3

Mild soreness, scrotal support, and limited movement. Most patients are back to desk work within 3 days.

Week 1 - 2

Suture review, return to driving, and resumption of routine activity.

Month 3 - 6

Repeat semen analysis (for fertility cases). Most patients see improved parameters.

How it Works

Your Varicocele Surgery Journey at Even Hospitals

Pre-Surgery ConsultationStep 1

Pre-Surgery Consultation

After consultation & physical examination, your surgeon performs a USG Scrotum with Doppler to confirm diagnosis, grade the varicocele, and evaluate fertility concerns. Clear instructions are given on pre-op preparation, fasting, and medications.

Day of SurgeryStep 2

Day of Surgery

Anaesthesia is given (regional or general), and the dilated veins are ligated via microscopic subinguinal or laparoscopic approach. The procedure usually takes 30 - 60 minutes.

Post-Surgery RecoveryStep 3

Post-Surgery Recovery

Mild swelling and discomfort are managed with painkillers. Most patients return home the same day or after 1-day observation. Light activity resumes in 2 - 3 days.

Follow-Up & Full RecoveryStep 4

Follow-Up & Full Recovery

A follow-up check-up is scheduled within 7 days. Semen analysis is done after 3 months if fertility is a concern. Full recovery in 1 - 2 weeks depending on activity level.
