Step 1Consultation & Diagnosis
After consultation, blood tests (PTH, calcium, vitamin D) and localisation imaging (sestamibi scan and ultrasound or 4D CT), your surgeon plans MIRP or conventional open exploration.
Step 2Surgery & Gland Excision
For MIRP, sestamibi is injected before surgery and the gland is removed through a 1.5 - 2.5 cm incision under gamma probe guidance with IOPTH confirmation. Conventional open surgery uses a collar incision to inspect all four glands. The procedure usually takes 45 minutes - 2.5 hours.
Step 3Post-Surgery Care
Patients are monitored for calcium and voice; MIRP patients are usually discharged the same day or within a day, while open surgery patients stay 2 - 4 days.
Step 4Recovery & Follow-Up
Recovery typically takes 5 - 10 days for MIRP and 2 - 3 weeks for open surgery, with serial calcium and PTH testing to confirm cure and taper supplements.
